What do we need from you?

  • Attach a Clinical history

  • Attach a Medication History

  • Confirm whether you want us to be part of a Team Care Arrangement and set up case conferences

How we will look after your patients

  • We will contact your patient and arrange a suitable time for an appointment

  • Ensure all paperwork is provided

  • Telehealth or face-to-face Consultations with Treatment Plan and Script

  • Ongoing progress checks – as needed
